Fill out the Partner Inquiry Form and Empowered Aging's HCP Director will reach out to set up an initial information meeting.
The HCP Program implementation will take 6 to 12 months from initial planning to the launch of the first CNA cohort and requires collaboration and effort from all partner organizations with the support and direction of Empowered Aging's HCP Director.
Partners are required to meet regularly and the basic steps include:
Identify HCP Partners and additional support partners
Collaboratively develop work plan
Establish class location & hire instructional staff
Obtain CDPH and other approvals as needed
Recruit students and launch initial cohort
Each County must demonstrate or meet the following requirements:
A workforce needs assessment shows that additional CNA training sites are needed in the County.
County demographics demonstrate that there is a need for job training with wraparound supports for individuals to successfully complete training programs.
The County has organizations willing to provided or develop wraparound support services such as food support, transportation, employment assistance, and/or childcare (can include referrals or access to already established county or workforce development programs).
There are willing participants to meet the core partner organizational structure: Older Adult Advocate, Educational Institution, Case Management representative, and Long-Term Care Industry representative.
