HCP’s Growth & Development

The Healthcare Career Pathway (HCP) addresses the critical shortage of direct care workers and includes comprehensive training for Certified Nursing Assistants - removing obstacles to success by providing one-on-one case management and wraparound supportive services.

Founding partners include Empowered Aging, Mt. Diablo Adult Education, and Opportunity Junction.

In 2022 HCAI granted Empowered Aging $5 million to expand HCP in up to 6 counties in California.

John Muir Community Health Fund

John Muir Community Health Fund (CHF) generously approved grant funding for the Healthcare Career Pathway (HCP) Program to plan and launch the HCP initiative at Mt. Diablo Adult Education in Concord, CA beginning in 2018. 

The HCP program created a regional response to address the shortage of CNAs in long term care facilities and the related quality improvement needs of those facilities. The HCP program also provided students with access to additional training pathways to greater economic success.

Partnerships & Expansion

With the success of the initial Mt. Diablo Adult Education site in Concord, CA, HCP was able to add a second CNA training program at Opportunity Junction in Antioch, CA. CNA training is still offered at both locations and has graduated 219 CNAs, as well as assisting graduates through the Alumni Program based at Opportunity Junction. 

HCP continues to partner with local organizations to provide wraparound supports to students and leverage the collective power of community. In Contra Costa County, food supports are provided by Loaves and Fishes and the Food Bank of Contra Costa and Solano.

The program’s success has been recognized by industry leaders and was included in the Master Plan for Aging in 2020 as a Local Model Program.
